We understand that looking after yourself means focusing on more than just your physical health. We can explore options with you to improve your wellbeing and help you to plan those next steps.
Please contact reception to book an appointment with your social prescribing link worker today. We also provide a monthly walk-in Social Prescribing Clinic held at the Family Centre, New Addington.

- Serious/unstable mental health
- Addictions requiring immediate support
- Under 18 year olds
- Has clinical needs
- Completion of benefits forms (PIP etc.)
- Blue badge applications
